Important Forms

Lodge Dues Payment Form - Use this form to pay your dues, payable August 1 and late on January 1 of the new year.

Member Demographics Form - Fill out this form and return it to the Council Service Center or to the Lodge Secretary at a lodge function if any change in your address, phone number, etc. occurs, or if you find a mistake in our information about you.

OA Troop/Team Representative Form - Every troop should have one OA Troop or Team Representative, who acts as a "middle man" between the lodge and the OA members in a unit, giving out key information and representing the unit at Lodge Executive Committee meetings and other lodge events. The OA Representative is a nationally recognized position, and counts toward the officer requirement for rank advancement. Unit Elections Form - Use this form if you have arranged with the Unit Elections Chairman to conduct a unit election. VERY IMPORTANT: Unit elections must be conducted according to very specific guidelines set forth by the National OA Committee. If you have not consulted the Unit Elections Chairman about your election, do so to avoid invalidation of the results.

Adult Nomination Form and Instructions - These materials should be used by Unit Committees to nominate an adult (21 or over) for membership in the lodge. Read the instructions carefully.
